New Patient Rules and Requirements:

• Must be at least 18 years of age.
• Complete an initial hygiene cleaning, x-rays, and doctor’s exam, then schedule a six-month re-care appointment.
• Follow the minimum required dental care as outlined by the doctor.
• After completing all necessary dental treatment, impressions will be taken for the whitening system; trays will be ready within 48 hours.
• Keep all scheduled appointments. Cancellations require a minimum 48-hour notice, and the original appointment must be rescheduled within 2 weeks.
• No outstanding bills with EZ Smiles Dental.

Existing Patient Activation Rules & Regulations:

• Must be 18 years of age or older.
• Comply with planned dental care.
• Adhere to the broken appointment policy.
• No outstanding bills with EZ Smiles Dental.
• At least a six-month history of punctuality and no missed appointments.

Lifetime Maintenance Rules:

• Continuous care as planned and scheduled by EZ Smiles Dental.
• Regular hygiene appointments every six months.
• Adherence to all payment and appointment policies of EZ Smiles Dental.
• A refill of whitening solution will be provided at each re-care appointment.
• Replacement of lost or damaged trays will incur a fee.

How to Use the Whitening Gel

Our take-home whitening system is designed to be easy and effective. Here’s how to get started:

1. Brush and Floss: Start with clean teeth.
2. Apply Gel: Put a small amount of bleaching gel into each tooth compartment of the provided trays.
